They probably did not show the trail sized parts of the road but went around it.
I saw a documentary once where they mentioned that parts of the road was widened so cars could use it instead of horses.

ad 1) Probably because it makes the book more interesting for a wider public. I.e. sells better. Without it it becomes an advanced book, with it it is also usable for a beginner…
